Ofek-4

Israeli satellite Ofek 4 failed to reach its orbit. Following a failure of the launcher during the 2nd stage burn, the satellite fell into the Mediterranean Sea. The satellite was supposed to be a prototype for future Israeli civilian satellite observation EROS-A Earth.
