First flight of PSLV. A guidance & navigation control software error resulted in one of the second-stage separation retrorockets failing to fire. Thus, the vehicle and its payload fell back to Earth 12 minutes after liftoff and crashed into the Bay of Bengal.

IRS-P1 was an experimental Earth observation mission undertaken by the Indian Space Research Organisation (ISRO). The mission's objective was to develop earth imagery using onboard instruments. Due to a malfunction in the rocket's final stage, the satellite deviated from its path and plunged into the sea.
