Vehicle disintegrated several seconds after liftoff with Autonomous Flight Termination System (AFTS) triggered, due to rocket being under-thrust & under-speed with respect to the flight plan, in turn caused by error in prediction of the fuel burning rate of the 1st stage motor. Maiden flight of the KAIROS launch vehicle by the Japanese company Space One.
