During the launch, one of the second stage engines shutdown prematurely. However, the four remaining second stage engines and the third stage were able to compensate by burning longer. The service module anomaly which prevented a moon landing was a fault of the spacecraft, not the launch vehicle.
Apollo 13 was to be the third moon landing in the Apollo program but was aborted due to an oxygen tank exploding. The crew returned to Earth safely.
