An overpressure event in the second stage caused the vehicle to fail during first stage flight at T+139 seconds. While Dragon survived the initial explosion, it impacted the Atlantic Ocean at high speed due to its flight software not accounting for this mode of failure, resulting in the parachutes not deploying.

CRS SpX-7

SpaceX CRS-7, also known as SpX-7, was a Commercial Resupply Service mission to the International Space Station contracted to NASA. It was planned to be berthed to the ISS for a 30 day mission, however, a failure in the Falcon 9 launch vehicle destroyed the vehicle in flight. 1,925kg of cargo were planned to be brought to the station, including IDA-1 (International Docking Adapter-1) which would be placed on one of the US Segment’s 2 Pressurized Mating Adapters (PMA) to adapt its docking system for future US crewed vehicles. This mission was flown by Dragon C109-1.
